When Tanjiro makes his way back home the next day, he returns to see an unconscious and wounded Nezuko lying in the snow and shielding Rokuta with her arms. Devastated, he realizes that the entirety of his family has been slaughtered, except for Nezuko.

in season 3 ep 4, there's a quick scene where Urogi just lays into him with his talons. It's the same scene where he gets the 3 scars across his chest. He also scratched his arms, legs, and shoulder blade during that attack. The location of blood splatter from those strikes matches the scars we see on him now.
