Physician Order For Life-Sustaining Treatment (POLST) is a form developed by the Georgia Department of Public Health pursuant to Official Code of Georgia Section 31-1-4.

The POLST form is medical order form approved by ethe Georgia Department of Public Health. It is meant for use by those who have a serious health condition or are nearing the end of their lives. It is completed by a person with decision-making capacity and their attending physician.

The Georgia Advance Directive for Healthcare (viewable here) combines three different healthcare decision making documents that previously existed. The first of these documents is the Healthcare Power of Attorney, which is now embodied in Part One of the Advance Directive.
