ADAP - Eligibility/Enrollment Process - New York State Department of Health
Upon determination of eligibility, an Enrollment Card is issued which may be used to receive care from an enrolled provider. The programs provide an automated eligibility verification system to allow providers to confirm participant eligibility status.

Show your card to participating health care providers to receive covered medical services at no charge. If you need them, you will receive home care services from an enrolled home health care agency at no charge ($30,000 maximum life-time benefit).
