Department of Children and Families (DCF) form , CF-FSP 5339 Medical Report for Prescribing Psychotropic Medication to a Child in Out-of-Home Care . The entire six page form is accepted.
If a court order is required to obtain authorization to administer psychotropic medication, for any medical procedure, the prescribing physician or psychiatric nurse must complete and sign the Medical Report form (CF-FSP 5339, available in DCF Forms).
Even if there have not been any changes to the current medications, a new 5339 is needed every year. Must include if parental consent was obtained and if not, why it was not obtained (TPR, parent refused, etc.).
All areas in the medication tab should align with what is in the 5339 for this medication and the medication documentations (to include any changes based on court orders, etc.). This is where you will list: 1. What medication is being taken 2. Who the prescribing doctor is 3. If medication is psychotropic or not 4.
As of September 1, 2011, Florida Medicaid, a division of the Agency for Health Care Administration (AHCA), requires informed consent from the parent or legal guardian of children age 12 and under who are prescribed psychotherapeutic medications.
