Applicant must have received a SJDB voucher furnished by a claims administrator. Apply by the deadline. RTWSP must receive application within one year from the date the SJDB voucher was served to the injured worker. How to apply: The application for the one-time $5,000 payment is available online.

Injured workers with a date of injury on or after Jan. 1, 2013 and who have received a SJDB voucher for said injury may be eligible for a one-time return-to-work supplement of $5000. All vouchers must be accompanied by a cover sheet prepared by the claims administrator containing notice of the Return to Work Supplement Program.

A one-time $5,000 Return-to-Work supplement is issued to an injured worker who meets the eligibility criteria. The amount of this supplement may be adjusted by the director of DIR based on further studies conducted by the director in accordance with Labor Code section 139.48.
