Identification and treatment of adenomatous polyps may be an important step in preventing the development of adenocarcinoma of the ileal pouch. In patients with FAP, ileal pouch polyps may vary in their size, number, and histology.

Restorative proctocolectomy is the most common surgical option for patients with familial adenomatous polyposis (FAP). However, adenomas may develop in the ileal pouch mucosa over time, and even carcinoma in the pouch has been reported.

Pouch cancer is a very rare event and may arise despite surveillance. Continued endoscopic surveillance is key in cancer prevention and early detection. Outcome of cancer cases is poor and management in a referral center should be advised with tumor board discussions.

Total proctocolectomy and ileal pouch-anal anastomosis is the operation of choice for patients with familial adenomatous polyposis. With this operation comes the risk of developing ileal pouch polyps.

Familial adenomatous polyposis (FAP) is caused by an autosomal dominant germline mutation of the adenomatous polyposis coli (APC) gene, on chromosome 5q21. The resulting phenotype affects multiple organ systems and is variable.

In 3741 patients with Familial Adenomatous Polyposis (FAP) the pooled incidence of pouch cancer was 0.01 (95% CI: 0.01 – 0.02). In UC most pouch cancers were found to occur in the pouch body (0.59 (95% CI: 0.29–0.84)).
