It was the Texas version of “POLST” (Physician Orders for Life-Sustaining Treatment), a national initiative to improve advance care planning processes. Since these processes are regulated by the state, each state must adopt its own form to be compliant with state law.

Nearly all states are working to grow adoption of POLST as a way to help patients, families and clinicians dealing with serious progressive illness and/or frailty. This map provides an easy way to access the website for POLST in your state or to contact your state leaders.
The POLST decision-making process is for patients who are at risk for a life-threatening clinical event because they have a serious life-limiting medical condition, which may include advanced frailty (www.polst.org/guidance-appropriate-patients-pdf).
