What is DA FORM 3434? DA FORM 3434 is used to record a variety of personnel actions for NAF employees. These actions can include appointments, reassignments, promotions, terminations, and other significant employment changes.

Form DA 3434 is used to notify personnel actions for nonappropriated funds employees within the U.S. Army. This form helps in documenting and communicating changes in employment status, such as appointments, promotions, transfers, or separations.

This form is used when a new employee is being in-processed or a current employee resigns or is separated from his/her position. This form is part of the permanent record and is filed on the right side of the OPF.
