Jun 27, 2019 · I started doing FMEA's 30 + years ago, (pre computer and hand written), the reasons for doing them are sound, but even now there seems to be much confusion as to what constitutes a "failure mode", or a cause, (of a cause etc.), resulting in more time spent discussing how to complete the formant and what goes in what box and completely missing the point of doing an FMEA in the first place.
