Erythema or edema of the skin should raise suspicion for periorbital cellulitis, dacrocystitis, stye, or blepharitis, which may have associated conjunctivitis.

Periorbital cellulitis occurs most commonly in children younger than 5 years. 14,15 Hallmarks of the clinical presentation include unilateral eyelid erythema, edema, and tenderness (Fig. 64-5). 1,7 The skin may display evidence of recent trauma or a break in the skin.

Generalized lid edema can be a diagnostic challenge to most of us in our daily clinical practice. Aetiology of lid edema can be due to trivial local conditions like a hordeolum, but sometimes it can be the herald of significant systemic diseases or neoplastic conditions.

Ocular symptoms include itching, burning, gritty, red eyes, dry or watery eyes, blurred vision, foreign body sensation, and crusting of the eyelids. Symptoms are often worse in the morning (Fig. 1). Patients typically exhibit eyelash loss and/or misdirection, a feature that is rarely seen with other types of blepharitis.
