A CFTM is a meeting where key people come together and collaborate, facilitated by a DCS Family Case Manager, to build a system of support and accountability for a family that has a child or children in the system.
DCS will utilize the CFT Meeting to engage with each family and child to create plans for assessment, safety, service delivery, and permanency. DCS will work with the family and/or child and placement (if applicable) to form the most effective team to assist with achieving goals.
