Dec 1, 2023 · This report describes a two-step surgical correction of an isolated left-sided congenital alar cleft in a 21-year-old male patient presenting with a 9 × 6.5 mm 2-sized cleft. Malformations of the alar structure and displacement of the lower lateral cartilage were observed.

The alar web in unilateral cleft lip nose deformity is the result of caudal overgrowth and migration of the alar cartilage. The caudal edge of the prolapsed lateral crus overhangs the cleft-side nasal aperture on its superomedial aspect, producing this deformity.

This study aims to quantitatively compare the shape, volume, and axis rotation of the cleft-side ala to the contralateral non-cleft ala in skeletally mature patients with UCL to understand how we may address alar asymmetry at the time of secondary cleft rhinoplasty.

We are presenting a case report of an isolated congenital cleft of the alar rim. The defect was closed by the use of a rotation advancement full-thickness flap. With this technique, both symmetry and desired thickness of the nostrils were achieved.
