During this transition period you may see either University of Michigan Health-Sparrow OR Sparrow Health System on patient communications and billing.

MySparrow activation code. Enter your activation code as it appears on your enrollment letter or After Visit Summary®. Your code is not case sensitive.

We need some information from you in order to create your MySparrow account. If you are under 18 years old, it could take up to one business-week for processing and verification.

If you have an email address on file then your MySparrow username will be sent to your email account. If you do not remember any of this information, or you do not have a valid email address on file, you will have to contact your MySparrow help desk at 517.364.4357 to help you regain access to your MySparrow account.

If you are not applying to become a job shadow or intern with UMH-Sparrow, please be sure you are filling out the correct application. Contact the Volunteer Services office at (517) 364-3606, option 1 with questions.
